What Is Biological Age? A Plain-English Explanation

Short answer

Biological age is an estimate of how old your body appears to be functioning, based on measurable markers such as blood biomarkers, fitness, sleep and metabolic health — as opposed to chronological age, which simply counts the years since you were born. Two people born the same year can have biological ages a decade apart because habits and physiology differ.

Biological age vs chronological age

Chronological age is fixed: it increases by exactly one year every year for everyone. Biological age is a model output. It takes inputs that change with health status — inflammation markers, glucose regulation, cardiorespiratory fitness, sleep quality, body composition — and expresses them as the age at which those values would be typical.

If your markers look like the population average for a 38-year-old and you are 45, a biological age model reports roughly 38. The number is a summary statistic, not a diagnosis.

How biological age is calculated

  • Blood-based models (for example Levine PhenoAge) combine markers such as albumin, creatinine, glucose, CRP, lymphocyte percentage, mean cell volume, red cell distribution width, alkaline phosphatase and white blood cell count with chronological age.
  • Epigenetic clocks measure DNA methylation patterns at specific sites and are typically run from a saliva or blood sample by a laboratory.
  • Lifestyle and functional models use fitness, sleep, nutrition, stress, body composition and habits — inputs you already know — to approximate the same idea without a blood draw.

What the number is good for

Its practical value is direction and prioritization, not precision. A biological age estimate is most useful when tracked over months against the same method, so you can see whether the direction of travel is right after changing sleep, training or nutrition.

Treat a single estimate the way you would treat a single blood-pressure reading: informative, but a trend matters far more.
